Install instructions: unzip the contents of this .zip to a directory of your choice
in your Train Simulator/trains/trainset directory. Then you can
add the caboose to your consists using the MSTS consist editor.

This car very likely  has a higher polygon count than many of the cars supplied with MSTS so may adversly impact frame rates on your machine.
